Full Job Description

Are you driven to tackling mental health-inequalities in our diverse multi-cultural homeless population in Southampton?, You will contribute to weekly multidisciplinary team meetings and work in partnership with colleagues to evaluate care plans.

You will be a positive role-model, reflecting a commitment to maintaining a culture of learning from incidents and valuing two-way feedback.

You will be responsible for providing supervision arrangements, to ensure practitioners are offered opportunities to develop their skills, knowledge and keep up-to-date with current practice.

You will have a good range of mental health knowledge and experience working with individuals who have experienced mental health issues.

You will be responsible for care-coordinating a small caseload of service users who are experiencing, or at risk of, homelessness who find it difficult to engage with mental health and drug & alcohol services. You will have experience in assessing and formulating individual intervention plans and outcome measures for adults with complex biopsychosocial problems and have current knowledge of NHS policies and practices in mental health. You will work in a strengths-based and recovery-focussed way to support services users to meet their mental health needs.

You will have excellent communication skills to enable you to foster relationships and engage with a variety of individuals from a diverse range of cultural backgrounds and partner agencies including CGL, Probation services and Southampton City Council local authority.

The transformation will happen in stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning across in May 2024. The final combination of Southern Health and Solent NHS Trust services is expected in late 2024, when our new organisation, to be known as Hampshire and Isle of Wight Healthcare NHS Foundation Trust, will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
