Full Job Description

You will provide a comprehensive assessment and triage service and, where necessary signposting or clear onward facilitation to a mental health pathway. Your referrers will include GPs, Police and Acute Hospital colleagues, self-referrals and referrals from carers. Service users will be triaged collaboratively and a decision will be made in consultation with the referrer and with the use of a clinical tool, the response time required. This comprehensive, collaborative triage system will lead to comprehensive assessment clear onward facilitation of service users to the most appropriate care following assessment. The post holder will provide evidence based mental health assessments and/or signposting to alternative service provision for people who present requiring a specialist urgent mental health assessment. The post holder may offer follow-up appointments within the agreed timescale for intervention as part of the on-going assessment process.

Registered Mental Health Nurse/ Social worker / Occupational therapist The Home Treatment team work in the community with people in acute mental health distress, they support people in their own home to avoid hospital admission and support early discharge from hospital. The team are looking for mental health practitioners that are highly motivated and enthusiastic. The successful applicant will need to be able to demonstrate excellent clinical skills. It is expected they would have a good working knowledge of mental health and community services. Excellent communication skills are essential with an ability to manage change and support colleagues around learning and development. You will need to be proactive, innovative and able to provide quality care, the service users and carers at the heart of everything you do.   • The Welcome Bonus is only available to candidates who are not currently employed (or who have not been employed in the last 12 months) by any NHS provider Trust in the Lancashire & South Cumbria (LSC) Integrated Care System (ICS) in a like-for-like role or core profession. For example, a Band 6 Occupational Therapist / Social Worker / Mental Health Nurse at another Trust in the LSC ICS would not be eligible for the bonus if moving to a Band 6 Occupational Therapist / Social Worker / Mental Health Nurse role at LSCFT or in a role such as Mental Health Practitioner Role that requires the same profession.These trusts are:o University Hospitals of Morecambe Bay Trusto East Lancashire Hospitals NHS Trusto Lancashire Teaching Hospitals NHS Foundation Trusto Blackpool Teaching Hospitals NHS Foundation TrustBank only workers for Lancashire and South Cumbria NHS Foundation Trust are eligible for the scheme if they have not held a substantive role in Lancashire
  • & South Cumbria Integrated Care System in the last 12 months (subject to all other terms and conditions being met). These incentives are subject to recovery clauses within the first 6 months of employment. This bonus is applicable from 1st July 2024 - 30th November 2024.

    The Barrow HTT are a 24 hr service working shift pattern of long days including nights, although happy to discuss flexible working patterns.
