Full Job Description

An exciting role has arisen for a Social Worker based at The Fells, to work alongside a passionate and vibrant support staff team. The Social Care team will aim to deliver a service to residents living within the service, ensuring access to support and advice, with necessary interventions undertaken in a timely manner, and also ensuring those leaving The Fells are ready and able to take on their own tenancy and are able to manage their needs within the community.

The service aims to reduce the need for statutory services and interventions and will work alongside other professionals to ensure a partnership approach to meeting service user needs.

The Social Care team will work with the service user(s) to identify social care goals as part of an overall plan. This will include developing and implementing specialist interventions for individuals or groups, in collaboration with the service user(s) and external MDT's.
