Full Job Description

The team provide short term intensive rehabilitation to patients in their own homes following hospital discharges and prevents hospital admissions for patients at home. You will have support from and work closely with the multidisciplinary team to provide patient centred rehabilitation programmes.

  • Relevant experience in a health and social care setting and base level of Occupational therapy / Physiotherapy knowledge
  • Knowledge of patient / client conditions relevant to community rehabilitation
  • Ability to work under own direction
  • Able to travel to meet the need of the role
  • The successful candidate will work alongside the therapy staff to promote independence and improve function for people recently discharged from hospital.,
  • Relevant NVQ Level 3 or equivalent achieved
  • Relevant experience in a healthcare setting and base level of therapy knowledge
  • Desirable criteria
  • Associate Practitioner Course or relevant experience
  • Experience Essential criteria
  • Extensive experience of working as a band 3 rehabilitation assistant in a community setting
  • Minimum of two years at a band 3 or equivalent
  • Desirable criteria
  • Experience of providing supervision
  • Awareness of relevant therapy techniques
  • Experience of working in adult social care
  • Knowledge Essential criteria
  • An awareness of NHS Plan, NSF and clinical governance priorities
  • Knowledge of patient / client conditions relevant to the community setting
  • Knowledge of multi-professional working
  • Understand the need for professional conduct
