Full Job Description

The successful candidate will work closely within the wider MDT to support patients in Wales with these rare conditions. The post holder will be expected to provide expert nursing advice and specialist support to patients, carers and professionals within the hospital and primary care setting as well as to school staff, children's services etcA Non-medical prescribing qualification would be desirable but not essential.Initial patient assessment and initiating a care plan for patients in partnership with the wider multidisciplinary team (MDT) Supporting patients whilst undergoing diagnostic tests Providing patients, families and staff with expertise (in-depth knowledge and understanding of general Inherited Metabolic Disorders and Lysosomal Storage Disorders) Liaise with metabolic laboratory to facilitate specific biochemistry and sample handling Initiating and implementing standards of nursing treatment and care for IMD patients Support nurse-led clinics Service developmentOther duties:
Liaise with community-based agencies and homecare providers. Actively involved in clinical trials. Network locally and nationally to ensure best practice.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
