Full Job Description

You will be required to be flexible, to move across sites according to the department needs and therefore transport will be essential.
Full and part time hours will be considered.
As a DCHS employee it will be your responsibility to ensure that your behaviour and values reflect those set out in the DCHS Way.
As a Physiotherapist working within our service you will:
+ Assess, diagnose, advise and treat patients with a variety of musculoskeletal conditions.
+ Be responsible for the management of patients on your caseload.
+ Maintain professional autonomy, being responsible for the management of your own patients whilst working within Trust and National guidelines.
+ You will be a key member of the outpatient physiotherapy team responsible for delivering a high quality physiotherapy service., To formulate and deliver individual treatment programmes based on sound knowledge of evidence based practice and treatment options, using clinical assessment, reasoning skills and treatment skills, eg respiratory therapy techniques, exercise therapy, patient education, and other techniques as appropriate.
To effectively use a wide range of verbal and non-verbal communication tools to negotiate and engage with patients across a diverse multicultural and intellectual spectrum. This will include patients who may have difficulties with understanding or communicating. For example, people with a learning disability, blind, deaf, dysphasic, cognitively impaired or who do not have English as a first language.
To appropriately convey complex information to other health and social care professionals to ensure collaborative working and the delivery of a coordinated multidisciplinary patient-centred service.
To formulate accurate prognoses and recommend best course of intervention.
Assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
Be responsible for organising and planning own caseload and others (e.g. Technical Instructors/Physiotherapy Assistants) and use of other resources to meet service and patient priorities, re-adjusting plans as situations change/arise, eg other colleague's sickness/annual leave.
To use acquired knowledge to identify those patients requiring on-going rehabilitation and refer to the most appropriate service to fulfil these needs in order to facilitate discharge.
Be responsible for maintaining accurate and comprehensive patient treatment records in line with DCHS NHS Trust and HPC Standards, including producing timely and informative written reports and discharge summaries.
Be responsible for collecting statistics as required by the clinical areas and DCHS for service level monitoring.
Participate in staff appraisal scheme and be responsible for complying with your agreed personal development programme and the progression of those appraised.

+ You will have previous experience of working as a physiotherapist, including working in musculoskeletal outpatients.

Derbyshire Community Health Services has a great opportunity for a Physiotherapist to join our Derbyshire Musculo-skeletal Outpatient Physiotherapy team, based in Belper, Ashbourne and High Peak and Dales.
We offer a supportive environment with senior clinical mentorship and leadership support across all of our teams.
We have an established mentorship and in-service training programme, offering CPD time. People joining the Trust would be eligible to join the DCHS preceptorship programme.
