Full Job Description

You will facilitate, empower and educate the families and the teams to enable holistic, expert and seamless care throughout their journey and intopalliative care planning and support the ongoing service development of the CNS model and core pathways.

You must have relevant experience, excellent teamwork and communication skills, be passionate about service development and improvement and dynamic in your thinking.
