Full Job Description

The role of the Neurological Conditions Assistant Practitioner / Nurse Associate will involve working alongside the Neurological Conditions Specialist Team (NCST) to support them in planning, coordinating and delivering holistic treatment for neurological patients in the community. The NCST manage a range of neurological conditions including but not exclusive to; Multiple Sclerosis, Parkinson's Disease, Motor Neurone Disease, Brain Injury, Stroke and Epilepsy. You will gain expert advice, support and training from the NCST, enabling you to develop your knowledge and skills in this specialist area. We are looking for someone who is proactive, with a passion for supporting the delivery of high quality patient care. You will have excellent organisational, communication and interpersonal skills and have experience or an interest in developing your skills in neurological conditions and/ or the community. If you are an existing Band 4 Assistant Practitioner or Nurse Associate, or newly qualified Nurse Associate we would welcome an application from you. Please note that all interviews will be held face to face in Milton Keynes., The post holder will work alongside the NCST to help coordinate, plan and deliver specific care plans/ treatments for neurological patients in the community. This could include a mixture of home visits, outpatient appointments or virtual appointments. You will also support the NCST with administrative tasks relating to patient care, such as managing and coordinating referrals to other services, organising patient clinics and multi-disciplinary team meetings and ordering of equipment. You will work collaboratively with all members of the NCST, Neurological Conditions Service, Milton Keynes University Hospital (MKUH) and Home 1st Planned Care services, as well as other internal and external professionals / services., Assist the Neurological Clinical Specialists and Neurological Clinical Specialist Practitioner to complete holistic assessments and care plans for patients, including liaising with all agencies involved and making any onward referrals. To implement care plans to meet patients clinical needs under the guidance of the NCST Participate in MDT meetings including supporting with organisation of MDTs and documentation of relevant clinical information Attend patient and carer support groups in the community and support with their delivery where appropriate Monitor patients psychological and physical wellbeing including using NEWS2, and take appropriate action when indicated To assess for and order equipment from the equipment provider and/or liaise with other services such as Community Occupational Therapy regarding patients equipment needs Participate in care record and other relevant audits For the detailed job description and main responsibilities of the Neurological Conditions Band 4 Assistant Practitioner / Nurse Associate please refer to the attached Job Description
