Full Job Description

Snowdon is a 14 bedded unit which specialises in the treatment of adults with physical and cognitive impairment following a recent neurological event or a long- term neurological condition. It is located within the South of England Rehabilitation Centre (SERC). Within this unit there are also two level 3 physical rehab wards and one level 2 neuropsychiatric rehab unit for patients who have acquired a brain injury.

You will work as a member of a multidisciplinary team alongside service users to promote independence and daily living skills, including personal hygiene, diet and fluids, encouraging them to engage in social or leisure activities as identified within their Care Plan. This role will assist and support qualified staff and carry out assigned tasks as directed, without supervision.

This role will provide support in management of risk, developing therapeutic relationships, supporting people to live meaningful and fulfilling lives.

Main duties of the job

To assist qualified staff daily in the provision of care and undertake tasks to facilitate the smooth running of the service as directed by the Service Lead.

Develop rapport with patients and treat all patients, carers an staff with dignity and respect.

Work with nursing staff, multi-disciplinary teams, and other allied health professionals.

Use interpersonal and communication skills to de-escalate and reduce aggressive behaviour if encountered from patients and carers.

Act in a manner to respect the customs, individuality, values and sexuality and spiritual beliefs of all patients.

Using judgement skills when seeking of feedback from patients to inform qualified staff to enable the development of services.

Following training and meeting competences undertake task delegated by qualified staff i.e., documenting patients care keeping records up to date on computer, carry out routine observations documenting findings and informing qualified staff.

To raise incidents appropriately via the reporting system in an accurate, objective and concise manner.

To ensure timely and accurate recording of patient information on patient systems.

Receive routine information and communicate to line manager /Clinical lead.
