Full Job Description

We are looking for enthusiastic individuals to join our Community Nursing Support Service (CNSS) Team in Portsmouth. The CNSS team is an integral part of the Community Nursing Localities team that provide Nursing care to patients within their place of residence. Working alongside the Out of Hours Community Nursing Team, your role as a Night sitter will be between the hours of 2200 until 0700 and will support patients who are in the last weeks of their life and who wish to die at home or in their place of residence. The service aims to support vulnerable patients and their families at home throughout the night to ensure continuity of care, aiming to support patients following unexpected events which would otherwise lead to distress for the patient, or possible hospital admission. You will ideally have experience of working within the Community; experience in End-of-Life Care/Personal Care would be an advantage and have excellent communication and interpersonal skills. You will be welcomed into the team and will receive regular Clinical supervision and support. You must be a car driver with access to a vehicle.Main duties of the job Work as part of the Community Nursing team, during the night, caring for patients in the last three months of life who choice is to die in their home or place of residence. Monitor planned care for patients whose condition is deteriorating, and care needs are increasing at the end of life and providing support and continuity for both patients and family/carers during the night.

  • Communicate with a range of people (clients, carers, and voluntary and public sector agencies relevant to patient care).
  • Support patients both physically and psychologically.
  • Assist patients and their cares with personal care as need arises during the night and implement planned care e.g., medicine prompts, mobility support, emotional and physical support.
  • Monitor patient progress using independent judgement to observe progress/deterioration in a patient's condition / situation during the night.
  • Carry out safe moving and handling practice, on a daily basis for moderately dependent clients, in the community, in accordance with planned programmes of care and Trust policies.
  • Undertake therapeutic handling with clients during personal care, transfer / mobility practice which may involve dealing with continence issues / body fluids.
  • Undertake monitoring of diet and fluids during the night shift.
  • Liaise with other professionals involved in the patient's care and / or treatment as agreed with registered professional.

    We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by the end of 2024.
  • Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities. The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust. Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on the day the trust is formed. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.

    £22,383 to £22,383 a year Please note for part time hours the salary will be pro rata.
