Are you a qualified Nurse Associate who wants to make a difference? Do you want to be involved in the continued development of an innovative integrated service? As an NHS Registered Nurse Associate of a well-supported general nursing team within Nottingham CityCare Partnerships Integrated Care Homes Service, you will bridge the gap between health care assistants and Registered Nurses; complementing the Registered Nurse role and working under an appropriate level of supervision from the Registered Nurse. Working as part of the wider multi-disciplinary team you will provide nursing interventions to a wide range of patients, by in reaching into Nottingham City Care Homes to deliver NHS services. The services overarching aim is to ensure that care home residents and staff have an improved experience through high quality care and access to familiar health professionals with the skills to meet their needs.

The nursing caseload comprises of adults living in residential care homes who have a nursing need.

To find out more about the role within this service we would urge you to come and shadow us for a day to see what we do and the benefits we can make to a patients life., The role of the Registered Nurse Associate is to support the Registered Nurses in providing nursing interventions to patients whose home is within a residential care home. This includes insulin administration, wound care, continence care, palliative support and EOL care.

As a Registered Nurse Associate within this integrated service, you will follow a competency-based induction and training pathway supported by a supervisor, to develop new skills and knowledge across all the elements of the service to enable a holistic approach to patient care, improving the patient outcomes.

The service plays a key role in implementing the Enhanced Care in Care Homes Framework by providing support to care home staff, enabling them to deliver optimal care to their residents. This includes delivering training and education sessions to staff within the care home setting to upskill and empower them to deliver optimal care to their residents. As well as developing joint handovers within care homes to enable proactive care and prevent avoidable hospital admissions.

The Integrated Care Home Service comprises of community nurses, dementia outreach nurses, AHPs and specialist nurse practitioners, and works closely with the wider MDT. This includes GPs, pharmacists, the care home staff and specialist services. We work closely with other healthcare providers and Local Authorities across Nottingham and Nottinghamshire and the care homes in the implementation of the Enhanced Care in Care Home Framework.
