Full Job Description

We have an exciting opportunity for a Nurse Associate to join the North West Leicestershire GP Federation to support our Hugglescote practice, either as a full time member of staff or part time from 22 hours and above. Our Nurse Associates are supported and mentored by our Senior Clinical and Leadership Team, and work as part of a multi-disciplinary team in a patient-facing environment., You will be a registered Nursing Associate with the NMC and will have completed your foundation degree. The PCN Nurse Associate will work closely with a multidisciplinary team to support the delivery of key priorities around prevention, tackling health inequalities and providing high quality patient centred care in line with the National Enhanced Direct Service requirements., In line with best practice and with support from the MDT, you will carry out various procedures to monitor and improve the health of patients. You will provide primary support to general practice staff and carry out Chronic Disease Management annual reviews delegated by a supervising Nurse or senior colleague.

You will work in collaboration with a range of other professionals, including GPs, practice nurses, pharmacists, paramedics, community nurses and other PCN services.

The North West Leicestershire GP Federation consists of 12 GP Practices covering 114,000. The GP Federation operates the North West Leicestershire Primary Care Network. We have a friendly, diverse and supportive multi-disciplinary team consisting of GP's (Partners and Salaried), Nurses (ANPs, Practice Nurses, Assistant Practitioners, HCAs), Paramedic Practitioners, First Contact Physio, Care Co-Ordinators and Social Prescribers. Ideally we are looking for a full time person to join our team, however exceptional part-time candidates should not be put off from applying but must be able to do a minimum of 20 hours per week. £25,000 to £27,000 a year Dependent on experience and pro rata for part time applicants
