Nurse Co-ordinator

An exciting development opportunity has arisen for a dynamic and motivated Registered Nurse to join our Early assessment team (EAT) as a band 6 Nurse Coordinator.

Working hours will cover 8am-9pm over 7-days, including Weekends and Bank Holidays. This position will be based at Pinderfields Hospital., This role will work as part of the Early assessment team, through a multi-disciplinary team approach working alongside colleagues from Therapy services, Social services and the Acute trust. Promoting positive staff behaviour, and facilitating the delivery of treatment to individuals, groups, and communities.

To work collaboratively to support the delivery and development of integrated care across the Wakefield District, working within the acute trust.

This is an excellent opportunity for a self-motivated and enthusiastic person to join Mid Yorkshire Teaching NHS Trust. As a Nurse Coordinator you will be working collaboratively with Occupational Therapist, Physiotherapist, Social Workers, and Acute Hospital staff to prevent unnecessary hospital admission at the Front Door, to facilitate earlier discharges and help people to get the support that is required to help them be cared for in the place they call home.

Experience is beneficial with knowledge of community services, and an interest in discharge and patient flow.

We are looking for an individual who has the confidence to challenge decisions at all levels, experience of working in a fast paced and high-pressure environment, and the ability to recognise the need to escalate complex discharges to senior management to prevent discharge complexities.

We have a reputation for providing a caring and supportive style working environment for our staff and you will find that we are determined to work towards improving health, preventing unnecessary admissions to hospital, and facilitating early discharges by caring for patients., The successful applicant will have good communication skills, IT skills, and a flexible attitude to working patterns and environments. Your work pattern will include shifts and weekends. You should also have a full clean driving license, and access to a vehicle for work purposes.

Experience of working in the community is essential.
