Full Job Description

The ambition of each rota'd shift is to have two MHP's and two HCSW. At times this will only be one, but support will be offered to minimise concerns around this. You will also be expected to move at times, to alternate aspects of Urgent care pathway, but this will be advised in advance of shift, if required. This is a 12.5 shift rota over 37.5 hours per week, day and night, therefore flexibility needed.

The successful candidate will work autonomously, taking responsibility for care, quality and clinical activity for a designated caseload of service users. To contribute to care quality performance and clinical activity by providing professional leadership and guidance, clinical advice and management in relation to the key dimensions of health care quality: Patient safety, The effectiveness of all health care interventions, Patient experience.

The post holder will be expected to foster a culture of networking in an open and transparent approach with internal and external partnerships and between all stakeholders. Being responsible for the assessment, planning, implementation and evaluation of programmes of evidenced based care to a group of service users according to changing healthcare needs and varying levels of complexity.

We have a fantastic opportunity as a band 6 Mental Health Practitioner based within the Mental Health Urgent Care Hub (MHUCH) and Place of Safety at Glenfield Hospital. You will also get to work very closely with CAP, CRT and MHLS. We are a very enthusiastic and supportive team and would be happy to receive applications from registered Mental Health Nurses, Social Workers, Occupational Therapists, Physiotherapists or Psychologists.

The MHUCH (Hub) was set up in response to a national request to divert patients who are in a mental health crisis and would ordinarily be asked to attend to A&E, may be taken to A&E by other emergency services or who may self-present at A&E.

This is on a referral basis and has no facilities for walk in or self- presentation.

The Hub provides an alternative location for patients to receive an assessment and minor clinical interventions for deliberate self-harm and overdoses (when it is clinically safe to do so) and operates by appointment 24/7.

The PSAU offers a secure environment for people who have been referred on Section 135 or 136 of MHA 2022. The MHP will work independendently , along with one HCSW ( Health Care Support Worker) to support a person who is considered to require an urgent MHA review to safeguard them. This will be undertaken , by allocated MHP on a rotational basis, in conjunction with role at MHUCH, Leicestershire Partnership NHS Trust (LPT) provides a range of community health, mental health and learning disability services for people of all ages. Delivered through over 100 settings from inpatient wards to out in the community, our 6,500 staff serve over 1 million people living in Leicester, Leicestershire and Rutland.
