Full Job Description

The Humbleyard Practice has an opportunity for an experienced nurse or ECP to join our nursing team. This is an exciting role comprising a professionally rewarding mix of outreach visiting of housebound patients, care home support and in-practice clinics. We are a flexible employer and can work with you to agree a working pattern that is mutually acceptable.

You will manage a caseload of housebound patients for whom, with their registered GP's support and direction, will provide advice and assistance with their continuing medical needs. You will undertake acute visits on a daily basis and provide care for patients from initial history taking, clinical assessment and diagnosis, liaising with the patient's GP, as required and the implementation of treatment plans. You will demonstrate safe, clinical decision-making and holistic care.

You will have the following:
Essential:

  • NMC or HCPC registration., We are a large GP Practice allowing structured staff development and we are growing the effectiveness of our nursing team to meet the needs of our patients and to meet professional aspirations of our nurses and other clinicians.     Have knowledge of the needs of patients with long-term conditions.

  • Possess excellent communication skills.

  • Be able work collaboratively with the practice team to meet the needs of patients, supporting the delivery of policy and procedures.


  • Desirable:
  • Independent Prescribing qualification.

  • Post registration qualification in long term conditions management.

  • MSc in Health & Social Care.
