If so, we have a fantastic opportunity for Band 4 Nursing Associate to bring their expertise and knowledge to assist in the development, implementation, and ongoing delivery of clinical care in our Frailty Unit and the Integrated Virtual Ward Clinical Team role out.

The role involves working very closely with the Acute Frailty and Virtual Ward multidisciplinary team (MDT) and is a pivotal role in ensuring all patients within the Virtual Ward receive the best possible care and service. This exciting new post is available for a dynamic nursing associate with an interest in the provision of care to patients suffering from frailty.

We are looking for Band 4 Registered Nurses to join our innovative Frailty team of 3 acute wards

We have an exciting opportunity for an enthusiastic, motivated Registered Band 4 Nurses, working closely with & under the direction of the Acute Frailty teams that provide appropriate services to people who are moderately/severely frail & have long term conditions.

This role is integral to the assessment of patient frailty levels & assisting with review of patients to allow the best access to care required for their support.

We are looking for someone who is adaptable, has a flexible can-do attitude with a passion for excellence & ability to work well both on their own & as part of a team.
