Full Job Description

It's an exciting time for our Community Respiratory Service as we continue to build on our successful model. CRS services work with patients in their homes across the High Weald, Lewes and Havens communities to support and manage their COPD and respiratory conditions.
Our multidisciplinary teams work flexibly to provide a 5-day service, Monday to Friday between 8 am and 4 pm. They deliver:
+ Detailed care plans, treatment management and oxygen prescribing in the home
+ A comprehensive pulmonary rehabilitation program
+ Support and advice in managing their long-term respiratory condition at home, You will deliver care within your proven competency as achieved during your Nursing Associate Training, and contribute to the planning, implementation and monitoring of patient care as part of the Community Nursing Team. You will recognise changes in a patient's condition and seek assistance from a Registered Nurse of band 5 or above and/or GP as appropriate.
If you are new to Sussex Community NHS Foundation Trust, you will join our preceptorship programme, through protected time and regular clinical supervision with dedicated registered professional support.
You will need to able to drive with a full UK licence with access to a car due to the geographic area covered by the team.
