Nursing Associate- Children and Young Adults Outreach Service

St Oswalds Hospice aims to provide the highest standard of clinical care for the children, young adults and families that we support. The purpose of this role is to act as an effective part of a multidisciplinary team providing specialist care to children and young adults with life limiting conditions and complex health needs. To assess, implement and monitor individualised holistic care. To deliver care, either independently or supervised ,within your scope of professional practice for the children and young adults, aged 0 to 25 years, who access the Children and Young Adults outreach and short break service. To contribute to ensuring that delivery of care is timely, well co-ordinated, effective and appropriate to need. Care delivery will take place within the short break unit as well as within community settings, as part of our developing Outreach Service.
