Job Summary The Nursing Team Lead plays a crucial role in assisting the Clinical Services Manager in managing and guiding the Nursing Team, ensuring the provision of a high-quality, patient-centred service. In this role, the Team Lead is responsible for supervising, managing, and motivating team members on a daily basis, fostering a collaborative and efficient working environment. As the primary contact for the Nursing Team, the role requires excellent communication skills and the ability to proactively address any challenges to ensure smooth operations. The Nursing Team Lead sets the standard by leading through example and actively engaging the team to meet shared goals. The post holder is responsible for ensuring the safe and high-quality delivery of services to patients, particularly in the management of long-term conditions, and is expected to uphold the professionalism and conduct required for the position. Job Responsibilities Managerial role: Create an inspiring team environment
with open communication. Set clear team goals and delegate tasks with deadlines. Oversee day-to-day operations and monitor team performance. Motivate and coach team members, addressing training needs. Listen to feedback, resolve conflicts, and organise regular team meetings. Formulate agendas for staff meetings and attend wider Protected Learning Time sessions. Assist in recruitment, induction, retention, and development of staff. Conduct staff appraisals and personal development planning. Monitor staff absence, manage return-to-work processes, and oversee leave entitlements, ensuring adequate team coverage. Organise extended hours rotas and ensure nurse access across all sites. Liaise with the Patient Participation Group and actively engage with wider practice issues. Liaise with other Team Leaders, the wider practice team, and external agencies to ensure efficient operations and minimise duplication. Clinical Role: The focus of the role is the delivery of evidence-based practice for
patients with long-term conditions and management and preventative nursing interventions to all patients. Although this is the main purpose of the post, the post holder will also be required to provide a practice nurse service including smears, wound care, and immunisation as required. Competently manage and support patients with long-term conditions (e.g., COPD, Asthma, Diabetes, Hypertension, CHD, Stroke). Implement, monitor, and evaluate treatment plans in line with the Quality Outcomes Framework and practice guidelines. Diagnose and manage chronic conditions using both drug-based and non-drug treatments. Proactively identify and support patients at risk of developing long-term conditions to prevent adverse health outcomes. Review and advise on medication for therapeutic effectiveness, ensuring patient adherence to prescribed treatments. Provide comprehensive assessments, screenings, treatments, and health education for chronic disease patients. Maintain accurate, up-to-date patient
records and adhere to legislative, policy, and protocol requirements. Ensure equipment cleanliness and stock control of vaccines, dressings, and other medical supplies. Deliver general practice nursing services, including smear tests, wound care, and immunisations. Attend and contribute to Nurse Team meetings, maintaining professional standards and working within NMC guidelines. Manage time effectively and prioritise workload to meet patient and team needs. General Responsibilities for all staff Adhere to practice policies and procedures and relevant legislation including the requirements of any professional bodies. Attend mandatory training as identified by the practice Highlight potential development areas.
