Full Job Description

This is a great opportunity for an OT with an interest in stroke who is looking to improve their skills in stroke management in a supportive and friendly team. The post is to cover a secondment.
The post holder will assess, plan and develop specialist OT treatment programmes for patients/clients with a wide variety of complex and highly complex physical, psychological and cognitive needs following Stroke. The aim of the community stroke rehabilitation team is to facilitate discharges from acute settings, provide specialist rehab to improve function, independence and living well after stroke and to prevent readmission to hospital, To assess, plan, implement and monitor care, facilitating best practice and
evidence based, person centred care
To be a resource and provide specialist advice, education and support to other health care personnel and people who have had a Stroke
To be actively involved in monitoring and developing the the Stroke service, to ensure patients receive sustainable high quality care that is effective and safe
To select and conduct standardised cognitive and perceptual testing to identify areas affecting functional ability and indicateneed for further need for further assessment/treatment
To be able to work independently, after a period of induction/education .
Contribute to the training and supervision of Occupational Therapists, Support Workers and other professionals within the multi-disciplinary team within specialism of Stroke
Be available to adapt and accommodate service demands, this may be in a challenging and intense situation.
