Occupational Therapist

Assess and provide high standards of initial assessment input to patients in the community.

Liaise with MDT members in treatment and management care plans and working closely

Support OT /PT with the recommendations for patient centred approach to achieve their rehab goals

Create, evaluate all care plans implemented and should have a sound knowledge in discharge planning and ability to make further referrals in to the community as required
