Full Job Description

Care to join us as an Occupational Therapist and be a part of the multi-diciplinary Team in providing an Intermediate Care Service to patients to promote faster recovery from illness, prevent unnecessary acute hospital admissions. Supporting timely discharge and maximise independent living within the recommended time frame of six to eight weeks. You will undertake holistic needs-led assessments in partnership with service users, patients and carersand explore innovative ways of collaborative working between general practice, health and social services and the private and voluntary sector in line with the Care Trust's integration agenda. You will take part in the Bay wide Intermediate Care weekend rota and late duty rota within the zone and may from time to time be required to cover other localities within the Trust. This role involves supervisory responsibility for a Band 5 post, Band 3 OT SWIC or ICSW or Assistant Practitioner as appropriate. You will have responsibility for managing a unit and community caseload. You will assess and treat patients in a variety of community settings; patient's own home, nursing homes, residential homes and acute setting.,

  • To work as a multi-disciplinary team within an inpatient rehabilitation unit contributing to joint decisions regarding the management of patients, including acutely ill patients referred by the GP or A&E/ED
  • To make a difference and help patients through their recover and returning back to health and independence
  • You may have supervisory responsibility for a Band 5 post or Assistant Practitioner as appropriate
  • You has responsibility for managing a community and IC caseload
  • You will assess and treat patients in a variety of community settings; patient's own home, nursing homes, residential homes

    Jack Sears House is a multidisciplinary; where a home first ethos working closely with the GP and community nursing services. The team will be based in a 29 bed rehabilitation unit. This will be a supportive and professionally stimulating environment and an exciting opportunity for the new post holders.
