The post is based in Hawkhurst Community Hospital. The role will include some domiciliary visits in the local area. We provide rehabilitation and moving and handling solutions for patients in their homes, and in the community.
Our teams work closely with our GP partners, Rapid Response, Community Nurses, Complex Care Nurses, Integrated Care, Health & Social Care Coordinators and other members of the multi-disciplinary team., You should be experienced, motivated and be able to work autonomously as well as part of a team. You must be an HCPC registered Physiotherapist with post-graduate experience. You must be able to travel around the Trust in a timely manner, so a car is essential
Your role will be:
To assess, develop and implement evidence-based treatment programmes for these patients within their own homes, community hospitals and other community settings
To be an autonomous practitioner with responsibility for own caseload working without direct supervision
To work as a member of a multidisciplinary/multi-agency team providing a high-quality service to adults who have a wide range of physical, medical or developmental problems
To provide support and information to patients, families and all agencies involved in the patients' care
To provide support, training and guidance to junior staff
