We are excited to offer a fantastic opportunity to join our Community Occupational Therapy Team as a HCPC Registered Occupational Therapist. This opportunity is currently a static post in our community team but we are looking for an individual who would welcome the opportunity at a later stage to rotate into other teams, for example; the Haywood Hospital and Home First (Discharge to Access) Service. Offering you a great experience and exposure to other specialities. You will work as part of our team of experienced Occupational Therapists, Assistant Practitioners and Assistants to provide multidisciplinary services to adults with a range of conditions across the different specialities. For newly qualified and experienced OT's we are delighted to offer you our extensive Preceptorship and Beyond Preceptorship Programme which will help support you in your developing career., o Keen interest & / or experience of working in rehabilitation services and can demonstrate a strong understanding and vision of specialist therapy and service needs.o To assess new service user referrals and care/support needs. The care/support plans may include the following: o Promoting independence/self-management and maintaining safety o Increased functional ability o Improvement in health and wellbeing including mental health o Interventions in disease/condition management o Prevention and reducing of health inequalities o Admission avoidance o Early supported discharge o Case management/ treatment/ care plans o Palliative and end of life careo Proactive in developing sound clinical skills in the assessment and management of patients in the rehabilitation and / or community environment.o To take accountability for allocated assessments, planning and implementation of care packages/ programmes of support within their caseload.
