Central London Community Healthcare (CLCH) is seeking two enthusiastic and skilled Occupational Therapists to join our dynamic Unplanned Care (Discharge to Assess/Rapid Response) Team in Barnet. What You ll Do:
+ Deliver Specialist Care: Provide high-quality community-based occupational therapy services to adults with complex health and social care needs, focusing on early discharge and hospital admission prevention.
+ Multidisciplinary Collaboration: Work closely with a team of healthcare professionals, including GPs, nurses, and social care partners, to deliver coordinated and holistic patient care.
+ Leadership and Development: Supervise and support junior and assistant staff, ensuring high standards of clinical practice and contributing to the development of a skilled and motivated team.
+ Service Innovation: Take an active role in service development, applying your expertise to improve patient outcomes and support new models of care. This includes assessing clients for assistive technology and planning complex discharges with a focus on value for money and best practice.

+ Qualifications: Diploma/Degree in Occupational Therapy with HCPC registration. Post-graduate training in Rehabilitation, Cognitive, or Neurological Therapy is essential.
+ Experience: Minimum of 5 years post-graduate experience, with at least 2 years in older people s services or rehabilitation. Experience working with complex cases and coordinating across agencies is highly valued.
+ Skills: Strong leadership, risk assessment, and problem-solving abilities. Excellent communication, team collaboration, and a thorough understanding of assistive technologies and moving/handling legislation.
