Full Job Description

As a Band 6 Occupational Therapist you will provide community based assessments alongside intervention planning for individuals deemed appropriate for the rehabilitation pathway. Using a holistic approach, you will provide an individualised intervention plan for service users, including but not limited to equipment provision, home exercise programming, activity planning, functional skills practice, carer involvement, environmental recommendations, advocacy work, motivation and confidence building and education and advice.
You will be working closely with our acute partners, such as Imperial College Healthcare NHS Trust and Chelsea & Westminster Hospital NHS Foundation Trust.
We will connect you with peers across the tri-borough of Hammersmith and Fulham, Westminster and Kensington and Chelsea to support you and to work on service improvement partnerships.
We will ensure you are able to follow the clinical and leadership development priorities identified in your personal development and appraisal plan.
We will equip you for mobile working and actively support flexible working practice when required and appropriate for the service.

Hammersmith & Fulham a great place to grow old. The Community Independence Service (CIS) has an opportunity for a Band 6 Occupational Therapist to join our rehabilitation team.
The Community Independence Service (CIS) provides planned and unplanned nursing and therapy interventions for older residents who have had a recent deterioration in their physical health and is run jointly between West London NHS Trust and CNWL Foundation Trust across the three inner London Boroughs.
