You'll work in partnership with residents and carers to set realistic goals and develop collaborative intervention plans, with regular reviews to monitor progress and adapt. Working as a member of the transdisciplinary team, you'll ensure that high quality individualised care is delivered, which ensures safety and a commitment to promoting recovery, wellbeing, and maximising independence/quality of life.
