Full Job Description

+ To triage and prioritise referrals into the service
+ To carry out holistic assessments of the functional abilities of people in the community whose needs can be complex, changing, and varied across the caseload.
+ To assess, plan, prioritise and provide outcome solutions including equipment, and minor and major home adaptations to a community-based client group.
+ To plan, organise and manage a community caseload with supervision.
+ To contribute to the service development of community-based therapy services.
+ To provide professional guidance and support to Community Assessment Officers and/or students

An exciting opportunity has become available for 2 Band 5 positions in the Adult Health and Social Care Community Occupational Therapy Team.
This innovative post provides the opportunity to work as part of the Community Occupational Therapy team in Salford providing assessments of peoples in a variety of community settings.
You will be responsible for your own caseload and have the opportunity to work closely with the wider community MDTs, adopting an integrated approach to patient care across Salford.
You will be a motivated OT who brings clinical skills, knowledge and understanding as well as excellent communication skills, enthusiasm and commitment to the team.
We are looking for a strong approach to team working and a drive for service development.
