Full Job Description

The community rehabilitation service provides care and support for people with complex mental health difficulties that have impacted on their daily lives. They explore and provide a wide range of goal-based interventions with people to support their recovery. Individualised programmes of care are developed to enhance independence, build meaningful activities and improved health and wellbeing. There is an emphasis on holding hope for people in their recovery. The programmes are offered in peoples own homes and can involve work in the local communities. The successful candidate will be expected to advocate for the people in their care and promote a recovery orientated focus throughout. The post requires lone working and will include 1 to 1 sessions and specific group work. The core hours of the post are 09:00 till 17:00 week days., Monday to Friday. There is an expectation for some flexible working between the hours of 08:00 and 20:00 including some evenings, weekends and public holidays if there is a service need. The service operates in three localities in East Kent, with each team consisting of a combination of community mental health nurses, occupational therapists and support from third sector workers. Collaborative working is essential, with close partnerships with Social care, individual care providers and Housing services. This coupled with the wider health servicesMain duties of the job To assist in identifying, developing and providing the Occupational Therapy contribution within the multi-disciplinary service in accordance with the needs of service users and the requirements of the service. To assess, plan, deliver and evaluate therapeutic interventions for identified service users. To manage a defined caseload of service users with occupational needs using evidence based, service user centred, hope and recovery, health promotion and relapse prevention orientated principles and practice.
