Full Job Description

· To provide a high standard therapy service to the RUH wards and Hospital at Home, promoting appropriate, timely and safe discharges.
· To provide ongoing therapy input alongside rehabilitation support workers as a member of the Hospital at Home Team until the patient is discharged from the service.
· To participate in weekend rotas.
· To perform advanced therapeutic assessment, diagnosis and treatment of patients who may have diverse presentations and complex physical and psychological conditions.
· To hold responsibility for own case load.
· Supervision takes the form of regular formal training, clinical reasoning sessions, and peer review and case conferences. Access to advice and support from a more senior physiotherapist/occupational therapist is available if required, clinical work is not routinely evaluated.
· To undertake all aspects of clinical duties as an autonomous practitioner.
· To report to the Therapy team leader weekly and help deputise in their absence.

Hospital at Home is RUH-led team that puts in place interventions and treatments that support patients in their own homes. This is a 7 day service where the team provides short-term intervention and continuation of medical recovery and rehabilitation at home. The aim of the Hospital at Home team is to provide hospital standard care in the person's own home to maintain the independence and dignity of our patients.
Hospital at Home is a multi-disciplinary team comprising of medical, therapy and nursing staff. We are looking for an enthusiastic OT who has experience of working with older adults, good communication skills and the ability to work as part of a team. Previous hospital experience is essential.
We have a high patient satisfaction rate with excellent feedback, also enhancing job satisfaction. This role is a good stepping stone between the acute hospital and community services and we are looking for flexible individuals who are able to meet the current challenges.
