Full Job Description

You'll be working as an integral member of the LAP team. To be based in Slough community to provide rapid OT assessments following a referral from the community hub. The OT will use their experience and expertise to clinically reason if service users can be supported by the integrated care team and use the other available community resources, to prevent admission. This problem-solving approach includes provision of equipment, signposting, organisation of short term rehabilitation services, recommending new or reviewing existing packages of care.
