Full Job Description

Provide specialist, high quality assessment & rehabilitation to improve health & independence, to enable earlier discharge from hospital or prevent hospital admission

Assess, manage & provide treatment to individuals with complex &/or multiple needs, working closely with the MDT providing supervision/support to other staff

Ability to deliver education regarding community rehabilitation

Main duties of the job

Be responsible for the assessment & provision of a specialised programme of rehabilitation, meeting the specific needs & agreed goals of your patients to facilitate their rehabilitation & optimise recovery potential

Manage a clinical caseload with autonomy

Monitor & evaluate treatment to measure progress/outcomes to ensure effectiveness of interventions & service delivery

Develop programmes of care to support discharge

Contribute to clinical leadership/supervision facilitating a culture of learning/reflective practice

Have high level communication & excellent interpersonal skills to negotiate & establish collaborative partnerships with patients & care professionals

Ensure compliance with clinical governance being responsible for ensuring Infection Control policy/procedures are followed

Assist in service development & projects as we strive to transform & modernise patient care

A vehicle to undertake travel is essential & the ability to work flexibly including evenings, weekends & bank holidays

To work as a Band 6 Occupational Therapist in NRS, in addition to BSc (or equivalent) in Occupational Therapy & HCPC registration, you will be able to demonstrate:

Ability to manage a varied caseload of people with rehabilitation needs in the community
