Occupational Therapist / Physiotherapist

CSH are currently looking for a Band 6/7 Physiotherapist or Occupational Therapist to join the UCR team. This is an exciting opportunity that will allow you to contribute to and participate in team objectives and service developments., You will work as a member of the multi-disciplinary Urgent Community Response (UCR) team. Undertaking assessments, clinical diagnosis and treatment of patients including telephone triage, multi-factorial falls assessments, and patients who may require crisis management and/or rehabilitation interventions to prevent acute hospital admissions and to support complex hospital discharges. These patients often have complex presentations.
