Full Job Description

An exciting opportunity has arisen for a B6 Mental Health Practitioner to join a newly created Home Intensive Treatment Service for older adults in Bury.
The team will provide an alternative to inpatient admission for Bury older adults with either a functional or organic condition by providing intensive treatment in the service users own home.
The team will also work to support clinical teams providing inpatient care to Bury older adults during the discharge process to ensure timely discharge and risk management and provide 72 hour follow up in the community.
The service will also provide a gatekeeping function to ensure prioritisation of referrals for inpatient admission and assessment of appropriateness for home treatment as an alternative.
The successful candidates will have the opportunity to contribute to the implementation and development of this newly established service in Bury.

- Supporting inpatient ward clinical teams during the discharge process to ensure timely and safe discharge of older adult Bury residents.
- To provide a gatekeeping function for all referrals requesting inpatient admission to ensure prioritisation and triage to ascertain if home treatment can be provided as an alternative to admission.
- To provide 72 hour follow up for all service users discharged from hospital., + To offer intensive home treatment as an alternative to inpatient care.
+ To facilitate early discharge/leave pathways to support discharge from inpatient services.
+ To provide assessment, risk formulation/management and assessment for service users.
+ To provide crisis interventions to service users.
+ To plan and deliver care for all service users.
+ To provide supervision for junior colleagues.
+ To actively participate in service development.
+ To participate in the overall development of the service ensuring that the principles of Clinical Governance underpin all developments.
+ It is expected that you will deliver care in accordance with all Pennine Care NHS Foundation Trust policies and procedures, the Mental Health Act and community care legislation.
Main Duties and Responsibilities
Clinical Work
+ To carry out a thorough assessment of the service users Mental and Physical health needs, including a risk assessment.
+ To develop individualised packages of care for service users experiencing a deterioration in their mental health, including signposting when necessary.
+ To provide intensive home treatment in service users home/community settings.
+ To provide crisis management and short term psychotherapeutic interventions as appropriate.
+ To develop and provide a supported discharge/leave pathway for inpatients.
+ To identify the need for protection in line with safeguarding children and the Protection of Vulnerable Adult Policy and to comply with statutory obligations.
+ To liaise and negotiate with other professions, statutory and independent agencies to ensure the best possible service is provided to service users and carers.
+ To evaluate systematically and record, outcomes of mental health assessments, adapting care on the basis of these outcomes.
+ To involve family and carers in the assessment and care planning with service users consent.
+ To provide education and appropriate self-help resources to patients specific to individual needs.
+ To maintain accurate and up to date clinical documentation on all service user contacts, ensuring confidentiality at all times.
+ To develop a thorough understanding of deliberate self-harm, suicide and possible management strategies.
+ To contribute to the establishment and review of policies, procedures and practice guidelines relating to the work of the Home Treatment Service and the interface between primary, secondary and specialist services.
Mental Health Services
+ To undertake evidence based practice.
+ To observe and practice within the legal requirements of the Mental Health Act 1983.
+ To maintain professional standards by engaging in continual professional development.
+ To be aware of professional accountability, with due regard to your Professional Code of Conduct.
Research and Audit
+ To ensure that PARIS records are efficiently maintained.
+ To contribute to audits as required, providing relevant data as required in line with performance monitoring targets.
+ To participate in research opportunities as and when appropriate., + To contribute to the ongoing training and development of non-mental health trained medical and nursing staff with regard to mental health issues.
+ To participate in clinical and managerial supervision in accordance with Pennine Care NHS Foundation Trust policy.
+ To provide mentorship, education and training to learners from a range of professional backgrounds.

Disclosure Barring Certificate check
All of our new starters, who are appointed to a post which requires a Disclosure Barring Certificate check (DBS), will be recharged for the full cost of the check (DBS checks for volunteers are free of charge).
An enhanced DBS check costs £38 and a standard check costs £18 (plus an administration charge of £3.50). If you are a substantive member of staff, the full amount will be taken from your first month's salary.
If you are a member of staff on our bank, you will need to repay the full amount from your first salary.
We encourage you to register with the DBS update service at an annual cost of £13 (this is mandatory for bank staff).
