This exciting opportunity awaits you within our Community Mental Health Team for Older People in South Somerset, based in either Chard or Yeovil., You will take on the rewarding role of being responsible and professionally accountable for a caseload within the community. You will work closely with individuals and their significant others who have multiple and complex needs, providing vital support and interventions.
