Full Job Description

We are seeking Band 6 MH Practitioner to work with the specialist OPMH part of our service, an exciting opportunity for the right candidate to expand their existing knowledge and skill set working in a Mental Health Liaison Team within a general hospital.

You will be an experienced Band 6 MH Professional, able to work autonomously with patients with either an acute mental health concern or worsening of a pre-existing condition, be able to conduct full, holistic biopsychosocial mental health assessments, undertake robust risk assessments and management plans and develop collaborative recovery focused care plans with patients and their carers. Working alongside our acute physical health colleagues at Queen Alexandra Hospital, an important figure in advocating for patients and promoting parity of esteem within the acute setting.

The team is made up of a full MDT - MH nurses, social workers, psychologists, psychiatrists, peer support workers and health care support workers. A rich learning environment, with daily MDT meetings, reflective practice, round table discussions as well as more formal teaching sessions. If successful candidates would like to expand their skills and knowledge across the life span there could also be an opportunity to develop knowledge of the all age part of our service, which support patients over the age of 18.

As a team we provide education to our physical health colleagues and there are opportunities for staff to be involved in this important part of our role., We offer a dynamic role to work with patients over the age of 18, with focus on OPMH patients with either an acute mental health concern or worsening of a pre-existing condition. The role will include:

Conducting full, holistic, biopsychosocial MH assessments.

Administering personalised interventions.

Providing tailored treatment and support.

Completing recovery focused care plans with patients.

Working collaboratively with acute physical health colleagues in Queen Alexandra Hospital to deliver holistic integrated care

As a registered professional you will automatically become a member of our Academy of Clinical Excellence (ACE) which has a specific focus on professional development and career progression of our nurses and Allied Health Professionals. As part of ACE's commitment , our development pathway for band 6's focuses on enhancing competence and confidence as a senior clinician and leader.

An excellent opportunity for experienced Band 6's wishing to extend knowledge and experience within liaison services or newly qualified seeking to progress your career within liaison. Applicants will be supported to gain competence by extensive in house training, bespoke supervision. If newly qualified you will join our trust preceptorship programme, progressing to a more senior banding within our service, once you have demonstrated you can evidence the required competencies.
