Have you got what it takes to be One You Lifestyle Advisor within Primary Care? Are you passionate abouthealth and willing to help other people to make the change to ahealthier lifestyle?One You Lifestyle Advisors work in their local communities, via GP surgeries, to help people who need a bit ofextra support to make the change to a healthier lifestyle. We providepersonal help and support to people on arange of issues including: Healthy eating, Exercise, Smoking, Emotional wellbeing and Advice on accessinglocal services.We need enthusiastic people to join our team! You don't need any formal qualifications, but you must bepassionate about health and able to use your local knowledge and greatcommunication skills to help people inyour local community. We have 1 part time (22.5 hours per week) positionavailable inthe Canterbury andCoastal locality, with the scope to cover Canterbury, Faversham, Whitstable, Herne Bay, Ash and Sandwich.Please note: wewill be conducting these interviews
virtually using Microsoft Teams. If you are successful forinterview, you will receive an email notification with a video link., One YOU Lifestyle Advisers (Primary Care) are expected to be kind and responsive but professional andinformative and contribute to the quality of the services provided withinthe community team across the healtheconomy by: Contributing to the on-going assessment, planning, implementation and evaluation of delegatedcaseload and recording alloutcomes on the appropriate systems seeking the support of senior/specialist staffas required or to support re-assessment.To undertake, and report back on delegated activities that have previously been signed competent. e.g. height,weight, and blood pressure. To undertake a variety of clinicalintervention tasks after achieving appropriatecompetencies delegated by a senior staff e.g. Health MOT's. Providing quality advice and guidance in relationto lifestyles/health improvement to support goal setting and self-management within scope of expertise., The data we collect helps us to ensure we are attracting diversity in our applicants, that all our colleagues have equitable access to career opportunities and supports us on our journey to being a Great Place to Work
