The Operational Pathway Lead For Stroke, alongside the Integrated Pathway manager, is responsible for the management and development of the Integrated community elements of the Sheffield Stroke pathway: the community stroke service (inclusive of early supported discharge) and the therapy team within SPARC the STH community rehabilitation unit., The Operational Pathway Lead is responsible for the delivery of:
Services aligned to the trusts aims: Deliver the best clinical outcomes, provide patient centred services, employ caring and cared for staff, spend public money wisely and deliver excellent research, education and innovation.
Achieving key national and local performance targets including SSNAP
Carrying out pro-active workforce planning activities to meet the needs of the service
To develop teams to meet national and local priorities
To support the transformation of new pathways of care and delivery of projects
To develop and maintain effective relationships with key stakeholders
The Operational Pathway Lead will deputise on a planned basis for the Integrated pathway Manager representing the community stroke pathway on agreed occasions.
