Full Job Description

An exciting opportunity has arisen to recruit to a new Operational Support Manager post, and we are looking for a motivated and enthusiastic team member to join us.

The Childrens & Young People Continuing Care team (GICB) is a child centred team responsible for assessing the needs and providing care to children and young people whose medical diagnosis means that their care needs exceed the level of provision offered by universal and specialist services.

The Continuing Care team works in line with requirements of The National Framework for Children & Young Peoples Continuing Care (2016) and is committed to promoting a collaborative approach, working closely with the families of the children and young people receiving support, and working with system partners including Gloucestershire County Council (GCC), NHS Gloucestershire Health & Care Foundation Trust (GHT) and a number of independent care providers within Gloucestershire and the surrounding area to fully meet the care needs of the child or young person to enable them to remain in the home environment for as long as possible., The post holder will provide business management support to the Service Manager and the various Children & Young Peoples Continuing Care CCC workstreams.

To work with and support the CCC team translating national policy and frameworks into local delivery for Complex Care and Personal Health Budgets (PHBs) assisting the development of robust processes which are consistently applied, maintained and audited.

Gather and collate monthly CCC data, escalate areas of risk/concern and triangulate information with other intelligence as acquired through multi-agency working.

To support the team to ensure that their workstream of programmes are planned and managed effectively.

Contribute to the development and changes in processes and systems to effectively analyse data submissions.

Responsible for the gathering and collating monthly/quarterly key performance indicators working with Clinical Leads to improve performance where required.

Monitor and collate data as required for any NHSE submissions and NHSE Benchmarking reports.
