Full Job Description

The post holder will assist in the delivery of high quality Osteoporosis /Fracture Liaison care within the Rheumatology outpatient services throughout the Trust. They will work alongside other clinical colleagues to provide expert care, treatment and support for this patient group.

The West Middlesex Hospital has a strong Rheumatology service, looking after a large cohort of patients led by Consultants and clinical nurse specialists. The individual will function within a field of practice that focuses on the needs of the patients, and their families within the remit of Osteoporosis.

The Osteoporosis CNS will promote excellence in nursing practice through the integration of expert clinical practice, education, and consultancy. The post holder will act as a resource to patients and colleagues and will be committed to utilising nursing research. The Osteoporosis CNS is responsible for the nursing management of patients with Osteoporosis who are referred to the service at West Middlesex Hospital. Their role will include provision of nurse led clinics for patients who are undergoing parenteral treatments for osteoporosis. They will also have the opportunity to learn about the Fracture Liaison Service and support the specialist nurse with this busy service.

Main duties of the job

To act as a role model in promoting equality of opportunity among staff in line with the Trust's Equality and Diversity Policy as well as conducting equality impact assessments on policy and/or service changes

To ensure local policies/protocols and guidelines are developed and reviewed to reflect best practice and that staff are informed of changes and work within these

To lead the review and re-design of patient pathways and services to ensure that these meet the changing needs of patients while optimising resources.

To run Osteoporosis led clinics

To work with the Fracture Liaison Service to assist with assessments of patients who have sustained a fragility fracture. To follow up patients, checking compliance and tolerance to medications which have been prescribed

To ensure patients are booked to be seen in a timely manner prior to parenteral treatments.

To work collaboratively with the Ambulatory Emergency Care unit to ensure patients are booked for Parenteral treatments

To co-ordinate high cost Osteoporosis treatments, liaising with pharmacists through the process.

Perform comprehensive assessment of patient health needs

Plan , implement and evaluate care delivery according to changing health care needs
