Full Job Description

We are working closely alongside a Local Authority in Liverpool to assist with the appointment of an Outreach Family Support Worker, on a 3-month contract, likely to be extended at clients discretion. Please apply with your CV for immediate consideration., Offer support on cases that become more complex and reach level 4. Support is continued on these cases to allow a smooth transition between early help and social care.
Provide intensive and targeted support to vulnerable children and young people. This includes bespoke packages and programmes. This also includes supporting adolescents with complex emotional and mental health problems.
Assess risk and need through early help assessments on cases that step down from Careline. Where appropriate adopt the role of Lead Professional to use common processes for children and families who require more support than universal services including the Early Help Assessment Tool (EHAT), Team Around the Family (TAF) and Lead Professional (LP) role.
Formulate Team Around the Family plans that reduce risk and need. Take the lead for the coordination of professionals and services to implement the plan and take the lead for reviewing the plan at the required timescales.
Use de-escalation methods to reduce challenging behaviour from adults and children and young people in families.

NVQ Level 3 in child care and education, NND, Health and Social Care qualification.
Experience of direct work with children, young people and their families
Experience of effective collaborative and multi-agency working to support children and families.
Experience of delivering outreach and home based support to children and families to address areas of identified need
