Full Job Description

We are looking for an enthusiastic Healthcare Assistant (HCA) to join our award-winning Overnight Nursing Team based on the coast. You will be part of the teams working to respond to urgent nursing care needs of patients in their own homes, supporting admission avoidance and preferred place of care.
1. Working alongside the registered nurse in providing urgent care to patients requiring care in the Community, including care of End of Life patients, those with catheter related problems & any other Urgent care needs.
2. Undertaking basic assessment of the patient ensuring accurate documentation and undertaking observations where appropriate. Able to demonstrate basic clinical assessment and convey any concerns to the Nurse
3. Maintain accurate written and electronic patient records using System One
4. Access and follow Trust Policies and Procedures as required
5. Skills in a variety of clinical areas such as palliative care and diabetes care would be an advantage, but full training can be given if required. We value the enthusiasm you bring .
6. You must be a car driver with access to your own car, and be prepared to drive across the geographical area. A good sense of humour is essential., We are seeking a highly experienced, confident, healthcare Assistant with some experience within the Community & the necessary skills in managing patients in their own homes.
Supports the Community Sister/Charge Nurse in ensuring the maintenance of high standards of nursing care.
Maintains the privacy, dignity and confidentiality of patients, relatives and colleagues at all times
Supports the Service in maintaining accurate documentation including the registering of patients on SystmOne and recording completed visits.
Responsible for monitoring, maintaining and requesting ordering of stock items.
Ensures that all equipment is kept in a safe condition and is ready for use when required, and that faulty equipment is reported and/or replaced as necessary.
Supports others in the application of local standards, policies and philosophies which further the philosophy of patient centred, individual and evidence based care for all patients.
Is aware of legislation and responsibilities under the Health & Safety at work act. Maintains a safe environment with regard to Risk Management policies and procedures. Is aware of and follows policies and procedures with regard to patients, themselves, other members of staff, equipment and the general public.
To report personal sickness and absence appropriately.
Participates in Supervision and Performance Development Review processes.
To undertake training and professional development as required, complete own Mandatory training and identify own development needs to keep updated as needed and maintain clinical and professional credibility

This is an exciting opportunity for a dynamic Healthcare Assistant to join the Brighton and Hove Overnight Nursing Service, covering Urgent Community Nursing Care for Brighton and Hove with cover to the High weald, Lewes and Havens area between 8-10pm.
This post is for 30 hours per week, working 12 hour shifts rotating about seven days a week.We are offering an opportunity to work two to three nights per week (24 - 30 hrs).
