Full Job Description

We are seeking a dynamic and committed Registered Children's Nurse with a special interest in children and young peoples diabetes to join our current Transition Diabetes team . This role would be to support our Diabetes Transition Service for our young patients aged 16-19yrs with both Type 1 and Type 2 Diabetes.

The CYP Diabetes service is for children and young people up to 16 years of age, with our transition service supporting up to 19 years. Our Transition diabetes team currently has one band 7 nurse. This role will be to work alongside the Transition Lead Nurse and the MDT. Although this role is primarily for the Transition Diabetes service, there will be some requirement to cross cover and work together with the Paediatrics diabetes services and MDT to provide a first class service to children, young people and their families., We currently have approximately 85 patients aged 16-19yrs with diabetes who we provide a service to, with approximately 35 receiving insulin pump therapy. You will work within the paediatric and adult outpatient setting, inpatient ward setting and also in the community to provide a high quality service to young people with diabetes and their families.

You will have post registered experience of working with children and young people with diabetes in a hospital or outpatient setting, and be interested in developing your skills and role further.

You will need to be flexible in your approach to work and in the hours that you may be required to work in the clinic and community in order that we can provide a service to children, young people and their families. The CYP Diabetes team currently provide an on call service at the weekend, which you will be expected to take part in.
