Full Job Description
An exciting opportunity has arisen for a 4/12 fixed term contract for a dynamic and committed band 4 paediatric flow coordinator to join our team in the paediatric department providing support to the nurse in charge of their allocated wards in dealing with patient flow. They will work alongside the Multidisciplinary Team (MDT) to ensure that effective discharge planning and communication are maintained in order to provide a smooth, timely patient flow and discharge from hospital., The postholder will support the ward team specifically with:
Working alongside the designated medical team to prepare EDN
Supporting correct referral pathways into CCN, CHAHT team
Liaising with community and MDT colleagues regarding follow up (Specialist nurses, Health visitor, School Nurse)
Ensuring that everything is ready the day before discharge so that the patients can be discharged smoothly the next day.
Communicating with relatives on the day of discharge to confirm they understand process and have all relevant information to facilitate safe discharge.
You will have experience as working with children ideally in a hospital setting. You will have a level 4 NVQ in Health and Social Care or other relevant qualification such as a diploma or equivalent experience.

You will need to be flexible in your approach to work the post will cover 7 days a week and will be from 09:00-21:00
