An exciting opportunity has arisen and Essex Partnership University NHS Trust (EPUT) are seeking to recruit to a Paediatric Liaison Health Visitor with an experienced, motivated and enthusiastic Health Visitor to join our friendly, supportive team, based in Rayleigh Clinic. The Paediatric Liaison Health Visitor role will develop, promote and facilitate appropriate referral and liaison between Acute A&E hospital services, walk in centres, Urgent Care Centre, Health Visitors, School Nurses and other community practitioners as required. Acting as a professional resource and link between Acute, Community and Primary Care services establishing good communications within these areas including wards, out-patients, A&E, SCBU and maternity services. The Paediatric Liaison Health Visitor will improve and facilitate robust methods of communication enabling professionals to become more effective in identifying vulnerability indicators in children, young people and families and ensure School Nurses/Health Visitors are involved in effective discharge planning arrangements of children as appropriate. The Paediatric Liaison Health Visitor will need to be able to work independently as well as within a team and be able to make decisions regarding safeguarding children.Main duties of the job To ensure facilitation of safe discharge needs of the complex patient work load inclusive of the Neo-Natal Unit and Paediatric A&E/PAU as required. Develop, implement and evaluate service discharge policies and protocols to ensure appropriate services receive information of children who have had an in- patient episode or attended Emergency Departments irrespective of the reason for admission or attendance. Participate in assessment, planning and provision of patient's needs before discharge. Ensure teaching and development of parents/carers and nursing staff. To ensure EPUT Community Services adheres to the Pan Essex Guidance for Managing Emergency / Unplanned Care Liaison Information. To be the named single point of access to children's community services for Southend University Hospital Foundation Trust (SUHFT) for children. To represent EPUT at agreed multi-agency / disciplinary forums, discharge planning meetings/ward rounds as required to meet the remit of this position. Ensure the timely transfer of information sharing from SUHFT hospital to community services as per the Essex Guidance. For children living within South East Essex, information from A&E, Neptune Ward and NICU will be scanned onto SystmOne and a task sent to the relevant practitioner within 2 working days, Monday to Friday excluding Bank Holidays.
