Full Job Description

The post-holder will have a good knowledge base relevant to this post including the assessment, management and leadership skills necessary for the role of a Clinical Nurse Specialist in Palliative Care. This individual will be responsible for providing expert clinical advice and support to patients with life-limiting illness, along with their families/carers, in collaboration with all members of the multi-professional team.

We are committed to assessing patient's needs in a timely fashion as soon as they arrive at the front door of our hospitals and ensure through fast track discharge that they receive this care closer to home as soon as possible.

Working in a team of medical staff, clinical nurse specialists and palliative care consultants healthcare support workers/complimentary therapists and AHPs, you would be contributing to the development and delivery of this vital service.

You would be part of a rotation working in all the 4 hospitals within the secondary care service in ABUHB. This will include the Grange University Hospital, Nevill Hall Hospital, Royal Gwent Hospital and Ysbyty Ystrad Fawr.

Main duties of the job

This individual will:

Effectively manage their own caseload of palliative care patients in secondary care, Monday to Friday 9-5pm and on a rotational cover for weekends.

Promote effective working links with both NHS and voluntary sectors to ensure a coordinated specialist service to patients with complex palliative care need along with their families/carers.

Support the Directorate in achieving the delivery of the objectives specified in the Welsh Government Palliative and End of Life Care quality statement.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
