We welcome applications from people who share our values and can help us deliver outstanding care in our local community., Coordination Duties: To work as part of the team for the Specialist Palliative Care Service liaising closely with clinicians to ensure a timely response to referrals. Taking and recording referrals electronically and allocating care packages under the supervision of the lead clinician and working alongside the integrated teams and a variety of organisations and care providers. Call Handling Duties: To deliver an effective, safe and competent call handling service for callers to the Specialist Palliative Care Service which includes patients, their representatives and Healthcare professionals, The role is interesting and varied, where candidates require good communication skills with the ability to interact professionally with patients, relatives and other staff groups:

  • Administrative support to clinical and non-clinical staff
  • Make and receive referrals from a number of agencies. Record referrals electronically on SystmOne and liaise with clinicians to ensure a timely response to referrals
  • To be responsible for working with team leads and clinicians revising and prioritising packages of care, as required
  • Communicate effectively to care staff working within the service regarding patient needs
  • Managing, monitoring and actioning tasks on SystmOne and emails received by the service
  • To carry out and prioritise a variety of administrative and clerical duties and undertake other general office duties
  • Provide service users and professionals with information, assistance and signposting, as required, To communicate with patients, families, carers and colleagues at an appropriate level providing clear information and guidance and dealing with patient's fears and lack of understanding. Exercising tact and judgement in dealing with and resolving routine enquiries, taking accurate messages and ensuring these are passed to the relevant person in a timely manner.
  • Work closely with the existing administrators across the organisation to provide ongoing administrative support to the Trust as a whole
  • Act as an administrator for new and ongoing tasks within the team
  • Support initiatives for the team
  • To create and maintain reports, spreadsheets and correspondence using a variety of computer packages including Word, Excel and Outlook
  • Support the management of absence process by completing and submitting absence forms as required
  • Manage the phones and output from calls
  • Processing, filing and distributing confidential reports and documents
  • Interact with employees at different levels of the organisation
  • Produce letters and arrange diaries
  • Maintain databases
  • Carry out any photocopying, scanning or filing, as required
  • To undertake any duties commensurate with the post
