Full Job Description
This post is key in order to continue implementing quality improvements in the provision of Palliative and EOLC for patients and their families, ensuring patient and care experiences are enhanced in order to achieve the best care in the right place., The post-holder will be responsible for the implementation, dissemination and support of the Personalised Care Plan for the Last Days of Life within the Trust through direct clinical practice and education/audit.
The post-holder will act as a Keyworker for designated palliative / EOLC patients, providing a link between patients and health care professionals across primary, secondary and tertiary care and act as an advocate for patients/families.
The post-holder will work with the Palliative Care Team to improve end of life care for cancer and non cancer patients and the EOLC training programme.
The post-holder will work with the Palliative Care Team regarding the EOLC tools to ensure they are embedded within practice.
The post-holder will work closely with the Acute Oncology Service/EAU Departments, Herts/Beds/Bucks Discharge and In-Reach teams to support the rapid discharge of eolc patients to their PPC.
The post-holder will ensure their role reflects the core components of the specialist nurse which includes: clinical practice, management, education, research and audit, taking into account the national EOLC agenda, quality markers and its impact on service delivery.
You will need to demonstrate experience working in a palliative care setting, with proven communication, organisational influence and teaching skills.
