Full Job Description
As the Community Palliative and End of Life Care Lead, you will support the Trust's end-of-life care portfolio, coordinating and leading all aspects of our end-of-life care programme in the Richmond community. In this influential role, you will bridge connections with healthcare and community partners, ensuring a seamless, compassionate experience for patients and their families. Your leadership will guide the Trust's strategy, policies, and governance framework to ensure our services remain aligned with national standards and deliver care that is safe, effective, and responsive., o Lead the development, maintenance, and implementation of the Trust's end-of-life care strategy, ensuring alignment with national guidelines and addressing local needs.
o Collaborate with clinical leaders across the Trust and partner organizations to drive a high-quality, coordinated approach to palliative and end-of-life care.
o Develop and monitor key performance indicators (KPIs) for quality assurance and performance evaluation.
o Support the governance framework for end-of-life care by coordinating clinical audits and ensuring the risk register is current.
o Facilitate training and professional development for clinical and support staff, fostering best practices in end-of-life care.
o Act as the Trust's clinical leader for end-of-life care, guiding the development of policies, pathways, and initiatives to enhance patient and carer experiences.
