Full Job Description

Our Parkinson's practitioners can provide information, advice and specialist support for people with Parkinson's and for their carers and families as well as healthcare professionals involvedin their care., To assess, plan, implement and monitor care, facilitating best practice and evidence based,person centred care within Parkinson's.To facilitate patient empowerment by enabling people with Parkinson's to manage theircondition effectively and independently preventing complications and improving quality of life.To be a resource and provide specialist advice, education and support to other health carepersonnel and people with Parkinson's, and contribute to planning, delivering and evaluatingprogrammes of education and development for Parkinson's careTo be actively involved in monitoring and developing the Parkinson's service, to ensurepatients receive sustainable high quality care that is effective and safeTo be able to work independently, after a period of induction/education with minimalsupervision.

They may require some support and will be expected to obtain this from seniorcolleagues, to make decisions which may be outside their scope of knowledge and practice.To contribute to the development and delivery of services for all people with Parkinson's andParkinsonism ie; Progressive Supra Nuclear Palsy , Multiple System Atro phy, Corticobasal Degeneration
